09-QUALITY ASSURANCE DOCUMENTATION

The University at Buffalo participates in a U.S. Department of Education program called Quality Assurance (QA). The mission of Quality Assurance is to help schools attain, sustain, and advance exceptional student aid delivery and service excellence.

Each year students complete the Free Application for Federal Student Aid (FAFSA). The student/parent responses on the FAFSA are processed and electronically transmitted to the University in a record called an ISIR.  The ISIR data are used to calculate the student’s expected family contribution (EFC), which is used by the University to determine the students’ financial aid eligibility.

The Department of Education has specific criteria it uses to select students for verification of the ISIR data. In addition, as a participant in the QA program, the University at Buffalo must also establish criteria for selecting students and verifying ISIR information. UB does not have to verify each and every ISIR that it receives; however, it must meet QA requirements, which includes performing a random sample of the entire aid population every other year.

If you have been contacted and asked to provide additional information, you were selected as a part of the 2006-2007 QA Sample. Please read the instructions carefully.

Complete the Quality Assurance Form - either for a dependent (PDF form) or independent (PDF form) student and submit copies of all required documents. Your financial aid for the academic year 2008-2009 (Summer 08, Fall 08, and Spring 09) will not process until you provide the information requested.
